By signing this you agree to the intervention necessary for tattooing. It is clear to me that a tattoo represents an intrusion into the integrity of my body and thus deliberate bodily harm. This procedure is carried out at my own request and request, and with my signature, I release the studio and tattoo artist from any responsibility for this intentional injury to the body. The liability of the studio and tattoo artist is limited to intent and gross negligence unless main performance obligations have been violated. Liability and claims for damages, including for complications of any kind and consequential damage, are therefore excluded within this framework. The undersigned declares that they have been sufficiently informed about tattoos, aftercare, and possible complications. Furthermore, the undersigned declares – not to be a hemophiliac, – not to have hepatitis, HIV, leukemia, cancer or diabetes, – not to suffer from contagious diseases, – not to suffer from a heart valve defect, – no blood-thinning medication or anticoagulants, such as Marcumar or similar medicines, – not to have any metal allergies, – not to have any acute infections, e.g. B. bronchitis, influenza or inflammation of the middle ear as well as inflammation in the mouth or tooth decay, – not to be suffering from psoriasis, neurodermatitis or so-called wild meat. The undersigned declares not to be under the influence of alcohol, medication, or drugs at the tattoo appointment. The care instructions have been read and understood. The signing person is either over the age of 18 or at least 16 years of age and has the fully completed “Declaration of Consent for a Tattoo for Minors” document. In the case of single parenting, proof must be provided. Untrue or incorrect information releases the tattoo studio or the tattoo artist from any responsibility.
